We value your time and ours. These policies help us maintain an efficient schedule so every patient receives the care they need.
48-Hour Cancellation Notice
We kindly request at least 48 hours’ notice for appointment cancellations or rescheduling. This allows other patients access to available appointment times and helps our office run efficiently.
Appointment Confirmation Required
All appointments must be confirmed prior to your scheduled visit. Our team will reach out to confirm your appointment in advance. Unconfirmed appointments may be rescheduled.
Same-Day Cancellation Fees
Same-day cancellations and missed appointments may be subject to cancellation fees. Please contact our office as soon as possible if you need to reschedule.
Deposits for Major Treatment
Deposits may be required for major treatment appointments in order to reserve appointment time and treatment resources. Deposit amounts will be discussed during treatment planning.

Payment Due at Time of Service
Payment is due at the time of service unless prior financial arrangements have been made with our office. Estimated co-pays and deductibles are collected at the time of your visit.

We accept cash, Visa, Mastercard, American Express, Discover, approved financing options (CareCredit, Sunbit, Wisetack), and insurance benefits.
Deposits for Major Work
Deposits may be required for major treatments and procedures such as implants, crowns, oral surgery, and Invisalign to reserve appointment time and treatment resources.
Treatment Estimates
Our team will review estimated costs before any treatment begins. Treatment estimates are provided as a courtesy and are not a guarantee of final charges.
Understanding Your Insurance Coverage
We accept most major PPO dental insurance plans and will help verify your benefits before your appointment. Please review the following information about insurance coverage and patient responsibility.
- Insurance estimates are based on information provided by your insurance company and are not a guarantee of payment or coverage
- Patients are responsible for any balance not covered by insurance after claims are processed
- Secondary insurance may be filed as a courtesy, but patients are responsible for following up and for any remaining balance
- We recommend verifying your benefits before your appointment so there are no surprises
New Patient Paperwork
New patient forms will be provided when you schedule your appointment. Please arrive 15 minutes early to complete paperwork, or contact our office to have forms sent in advance.
